Title: Rapid Re-Housing Case Manager Department

Department: Family Rapid Rehousing Program       

Reports to: Family Services Manager

FLSA Status: Non-exempt

Budgetary Line Responsibility: Not applicable

Salary Range: $60,000-$62,000 annually

Shift: 9am-5pm with one weekend & one evening

 

SUMMARY

EOCP’s service model has the following components: Housing First, Cultural Competency, and Trauma Informed Care. The Case Manager responds to the immediate and ongoing needs of families experiencing homelessness who qualify for housing placement and/or various forms of short- and long-term rental assistance that EOCP and funding sources- HSP offer. This position works directly with clients to identify and then apply for units of housing that they feel they can afford once our short to medium-term financial assistance ends.

 

This position will require a highly organized professional who is able to navigate a caseload of families with complex needs and histories of trauma while effectively multi-tasking and working with a small team of dedicated providers. This position educates clients about community resources while ensuring people are connected to the most appropriate resources needed to locate and maintain a new unit of housing successfully. This position reports to the Family Services Manager.

 

 

Essential Duties and Responsibilities

·        Assist program participants with housing location and navigation, credit checks, lease applications, and moving arrangements.

·        Meet with program participants both in the office and via in-home visits to ensure they are successfully navigating their new unit of housing.

·        Attend scheduled meetings related to program operations, case conferences, training, and participants and meet regularly with outreach partners.

·        Attends weekly Supervision.

·        Maintain accurate and timely documentation of daily activities in HMIS for households and closely track households identified for program eligibility. Note that documentation policies may change based on agency and funder standards.

·        Submission of ongoing reports examples: client trends, housing trends, etc.

·        Collaborate and respond in a timely manner with landlords, property managers, and housing locators so that housing opportunities are not missed, and concerns are addressed.

·        Assists and acts as an advocate for program participants in locating and retaining appropriate housing. This may include assisting with rent and security deposit negotiations and payments, the move-in process, completing housing inspections and obtaining needed household items, and other tasks related to maintaining clients' independent living.

·        Auditing of case files

·        Maintain and exercise confidentiality standards of HIPPA compliance.

·        Coordinate ongoing support in the following:

  • Employment and Training Needs
  • Needs of Youth and children e.g. tutoring. Summer camps and child care.
  • Financial literacy and budgeting e.g. credit repair

 

 

Qualifications

·        Equivalent to a four-year degree from an accredited college or university in sociology, psychology, counseling, or a closely-related field and three years of experience in mental health, social services, or a community services organization. Significant experience may be considered as a substitute for education.

·        Minimum of three years of experience working in the housing field with strong knowledge of the Alameda County housing market, geography, and housing law; experience in property management a plus.

·        Experience working in a social service setting with previous knowledge of rental assistance programs and activities.

·        Ability to be empathic and compassionate with clients combined while adhering to appropriate professional boundaries

·        Ability to navigate public assistance, social service programs, and public housing practices to help coordinate care and communicate client needs effectively.

·        Meticulous and in-depth record-keeping via online databases. Previous experience in HMIS database systems is preferred.

·        Valid California Driver’s License and access to a personal automobile.

 

Language Skills

·          Ability to speak clear, concise English;

·          Spanish or sign language as a second language is preferred.

 

Mathematical Skills

·          Strong basic math skills required at least at the high school level;

·          Must be able to perform mathematical calculations with a high degree of accuracy.

 

Reasoning Ability

·      Must be able to exercise good judgment while monitoring residents' adherence to

 program' guidelines;

·        Must exhibit good critical thinking skills to be used to solve problems;

·        Ability to utilize multiple sources of information to create comprehensive individual service plans for residents;

·        Must be a flexible thinker and doer, must have a team work approach to working with families.

 

 Computer Skills

·        Proficiency in using computer software to carry out tasks. Ability to use Microsoft Word to perform key job duties is required. User knowledge of Excel, database programs, and the internet/e-mail is preferred.

 

   Certificates, Licenses, Registrations

·        Must have current CPR and Basic First Aid certificates.

·        Valid California driver's license, insurance and access to an automobile. Proof of insurance and auto registration required.

 

    Physical Requirements

·        Must be able to dress neatly with a professional appearance when reporting to work.

·        This position requires an annual tuberculosis test;

·        Must be able to work under stressful conditions.

 

    Work Environment

The position requires an ability to work in a diverse and stressful environment with families that have multiple issues. Work schedules are flexible according to facility and client needs and can include night and weekend hours.
